Description Calcifediol is a major circulating metabolite of vitamin D3 and acts as a competitive inhibitor with an apparent Ki of 3.9 μM. It is produced in the liver and is the best indicator of the body’s vitamin D stores. It is effective in the treatment of rickets and osteomalacia both in azotemic and non-azotemic patients. Calcifediol also has mineralizing properties. According to MedlinePlus the normal range of calcidiol is 30.0 to 74.0 ng/mL. Calcifediol suppresses PTH secretion and mRNA (ED50=2 nM), but is several hundred times less potent than 1,25(OH)2D3. The lower potency of calcifediol correlated with its lower VDR affinity. bPTCs converted 25(OH)D3 to 1-hydroxylated metabolites, but the rate of conversion is low. In the pooled analysis, higher circulating calcifediol concentrations were statistically significantly associated with decreased colorectal adenoma risk (highest vs. lowest quartile odds ratio= 0.59, 95% confidence interval: 0.41, 0.84).
